Doctors discover key to preventing Crohn’s complications in children

A recent study shows that anti-TNF drugs significantly reduce the risk of anal fistula in children with Crohn’s disease. By preventing these painful complications, early treatment with infliximab and adalimumab can enhance quality of life and reduce the need for expensive surgery.

A recent study published in the journal Gut suggests that a specific class anti-inflammatory drugsMedications, known as anti-TNF (tumor necrosis factor), may help prevent anal fistula – one of the most debilitating complications of Crohn’s disease in children. Anal fistulas are abnormal tunnels that form from the inside of the anus to the skin outside. These painful, open-draining wounds can increase the risk of infection and organ damage and require aggressive treatment. The study shows that children who were given anti-TNF drugs after their diagnosis had an 82 percent lower risk of developing these troublesome fistulas.
Crohn’s diseaseA chronic inflammatory condition of the gastrointestinal tract, affecting approximately 40,000 children in the United States. This disease causes symptoms like diarrhea, cramps, stomach pain and fatigue. One in three children with Crohn’s disease will develop anal fistulas, which can be physically and emotionally challenging. These fistulas often require surgical intervention, and about 70 percent of children with this condition will require at least one surgery, sometimes resulting in the need for a colostomy bag.
The new research tracked more than 900 children who were recently diagnosed with Crohn’s disease. The study found that children who were given anti-TNF drugs such as infliximab and adalimumab early on in treatment had a significant reduction in the risk of developing anal fistula. These drugs work by blocking TNF, a substance that triggers inflammation, thus helping manage the disease and prevent complications.
This effect was especially significant in children who already had symptoms such as large skin tags, ulcers, or cracks around the anus. The presence of these lesions increases the likelihood of anal fistula, increasing the risk fourfold. However, in these children, anti-TNF drugs effectively reduced the risk of developing anal fistula by 93 percent.
While anti-TNF drugs are more expensive than other anti-inflammatory treatments for Crohn’s such as azathioprine and methotrexate, the results suggest they may be a worthwhile investment. Preventing the development of anal fistula and its subsequent complications can ultimately avoid the high costs of surgery and other treatments associated with the condition. The study’s lead researcher, Dr. Jeremy Adler, clinical professor of pediatric gastroenterology at the University of Michigan, emphasized that it is far better to prevent these complications after they occur than to try to treat them. He explained that it is extremely difficult to effectively address these complications once they develop.
For healthcare providers, the findings reinforce that anti-TNF drugs are not only more effective in preventing complications, but also more cost-effective in the long run. For families, this presents the challenge of choosing the right treatment, but evidence suggests that anti-TNF drugs provide the best results in terms of preventing serious complications and improving the quality of life for children with Crohn’s disease.
Symptoms of Crohn’s disease, beyond anal fistulas, can include fatigue, weight loss, poor growth in children, and persistent abdominal pain. In severe cases, it can also lead to complications such as bowel obstruction, perforation, or stricture. Therefore early intervention with anti-TNF drugs may play an important role in reducing the risk of these complications and improving the overall well-being of affected children.
