Newbornâs death âdue to gross failures of midwivesâ

A newborn child died due to gross failure of three midwives to provide basic medical care, a corner has ruled.
Ida Lock was born on 9 November 2019 at the Royal Lancaster Information (RLI), but died a week after suffering from a severe brain injury due to lack of oxygen.
After five weeks of interrogation at the Preston County Hall, Koronor James Adle concluded that Ida died due to the failure of the baby, when the baby was âimmediately clear when she was in crisis that she was in crisisâ and the lead midwifeâs âfull failure to provide the Basic Neanatal Rejuvenationâ.
He said that âthere were eight missed opportunities to change the clinical course of IDA.

The inquiry heard that the IDA was shifted to a intensive care unit in the newborn unit of Royal Preston Hospital, where he died on 16 November 2019.
The hearing earlier heard that April 2020âs April 2020 report identified several failures in the care of the IDA of Independent Healthcare Safety Investigation Branch (HSIB) which contributed to his death.
The review states that Sara Robinson, the mother of the Midwives IDA, failed to identify the fetal heart rate abnormally after participating in early labor.
After the birth of the IDA was ineffective revival, HSIB concluded.
But in January 2020, the internal âbasic reasons analysisâ before the hospital did not find any problem and praised the âgreat harmony and communicationâ shown by the employees in the delivery suit.
After reading the reports opposite âNight and Dayâ, Idaâs parents Ms. Robinson and Ryan Lock complained to the university hospitals in the Morkembe Bay NHS Foundation Trust (UHMBT), which runs the hospital.
âFully disabledâ
Dr. Giving her conclusion, Adle said: âIda was a normal child, who died due to lack of oxygen during her delivery, which was caused by the gross failure of three midwives, which provided basic medical care to give her immediately to provide her basic medical care when it was clear when she was in trouble.â
And he said that the âfull disableâ failure of Midwife, the head of his death, contributed to providing basic newborn revival during the first three-half minutes of his life.
This, he said, further contributed to the babyâs brain damage.
UHMBT was the theme of a harmful report in 2015, with a âdeadly mix of problemsâ in one of its maternity units at the Furnace General Hospital, which led to unnecessary death of 11 infants and a mother between 2004 and 2013.
Dr. The Morekamb Bay Investigation headed by Bill Kirkup exposed a series of failures âat every levelâ, from the unit itself to those responsible for regulating and monitoring the trust.
