Syphilis cases have increased in England, but gonorrhea cases have gone down, showing new figures.

The new cases of sexually transmitted transition syphilis have increased again in England, with a trend in the early 2000s.

While the total number of people suffering from gonorrhea has fallen, there has been a significant increase in the number of cases where the transition is drug resistant, the new UK Health Protection Agency (UKHSA) data show.

Health experts say that this is a real concern, although the actual number of drug resistant cases is very low.

The NHS recently announced the rollout of the world’s first vaccine program to protect from gonorrhea, aimed at mainly in gay and bisexual men.

Overall, in 2024, England had 9,535 diagnoses described as the early phase syphilis, 2%at 2023.

But the overall figure for syphilis, in which the late-stage is called syphilis, or complications from the infection increased by 5% to 13,030.

Gonorrhea figures show a more complex picture.

Overall, 2024 saw a decline of 16% in gonorrhea cases, with 71,802 diagnosis in 2023 compared to 85,370, with the biggest decline between 15 and 24 years old.

A monitoring and prevention scientist Giulia Habib Meiggi urged to be cautious on the fall for sexually transmitted infections in the UKHSA.

“This is the first year in the last few years where (number) has actually gone down,” she said.

“This is still the third highest number of cases we made in a year in recorded history, so it is good news, but this does not mean that it will remain like this.

“It is clearly important for people to tested regularly and use condoms with new partners.”

Drug resistant gonorrhea on growth

In contrast, there is acceleration in the diagnosis of antibiotic-resistant gonorrhea.

Some strains of bacteria that cause disease, now do not respond to the first row treatment, antibiotic safetrexone.

With 14 cases reported over the first five months of 2025 compared to 13 cases for the entire 2024, the number itself is low.

But six of those 14 cases have been largely drug-resistant this year, meaning that they were not only resistant to safetrexone, but also the second line treatment options.

Most Ceftriaxone resistant cases were combined with the Asia-Pacific region or with travel, where the levels of Ceftriaxone resistance are higher.

UKHSA scientific enforcement Narayanan described the trend as “worrisome”.

“The number is still small, but the reason for this is worrying, because once the resistance in gonorrhea becomes spatial, it becomes extremely difficult to treat it, because seftriexone is for us.

“We want to ensure that the spread of resistant strains is as low as possible and for a long time we can use Ceftriaxone to treat it,” he said.

What are the symptoms of syphilis and gonorrhea?

Increasing concern over drug resistance

The number of sexually transmitted infections remains high, warning the UKHSA, with effects mainly felt in children aged 15 to 24, gay and bisexual men and some minority ethnic groups.

But the increase in drug resistant cases of gonorrhea is a real concern, amidst extensive concerns around an increase in antimicrobial resistance.

The World Health Organization describes anti -antimicrobial resistance as an issue of global anxiety and is one of the greatest hazards for global health.

It threatens our ability to perform life-saving processes, including chemotherapy, caesarean section, hip replacement, organ transplant and other operations for cancer.

This is why, even though only 14 cases of drug-resistant gonorrhea have been identified this year, health experts urged anyone to use a condom to have sex with new or casual partners and to test regularly, whoever has their age or sexual orientation.
